Holistic needs assessment

A cancer diagnosis can affect many aspects of your life.   A Holistic Needs Assessment (HNA) is one way of identifying your concerns and what is important to you but also can help the health care professionals, involved in your care, understand this too.


Cancer care review

The cancer care review (CCR) is a conversation between a patient and their GP or practice nurse about their cancer journey. It is essential to personalised care and it is an opportunity for patients to:

  • Talk about their cancer experience and concerns.
  • Understand what support is available in their community.
  • Receive the information they need to begin supported self-management.

End of treatment summary

This is a discussion between you and your clinical nurse specialist. It takes place as your treatment is coming to an end. You will discuss your follow-up care.


Cancer registry

When you are diagnosed with cancer in the UK, some information about you, your diagnosis and treatment is collected in a cancer registry.  This information is used to plan and improve health and care services.  Your hospital will usually give this information to the registry automatically.

At Northumbria, we take active part in clinical trials and research studies.  Speak to your consultant or cancer team to see if you are eligible for a clinical trial.

Cancer quality of life survey

The cancer quality of life survey is a national survey which is run in partnership with NHS England. The survey is for people with a cancer diagnosis. It will be sent to you 18 months following diagnosis.

The survey’s aim is to find out how quality of life may have changed for people diagnosed with cancer. Your answers will be compared with information about their cancer diagnosis and treatment.

This will help the NHS see where care is working well or not so well, and if any new services are needed. This will help the NHS to improve the way in which it supports people to live as long as possible.
