Our Community Services Business Unit and Care of the Elderly team at Northumbria Healthcare are hosting a frailty event on Friday, 6 June.
The day is aimed at care home or frailty leads across primary care and we would like to see a representative from each practice across Northumberland and North Tyneside.
A full timetable is being finalised, aiming to be from 9am to 4pm with lunch included. Topics will include a mixture of clinical learning, case discussions and service updates around virtual wards and elderly assessment pathways.
